Planned Intervention

Planned intervention refers to the set of decisions regarding the various actions that will be developed within a center, either in the programming of activities at the care group level or individually on a personal level.

Internal Rules

Internal rules are outlined in the document that reflects the general rules of operation of the center, setting the terms of the relationship between residents and the center.

Resident Rights

  • Access to schools and care services without discrimination on grounds of sex, or any other personal or social circumstance.
  • To be informed and consulted on any matter that may affect or interest them as users.
  • To receive the necessary attention, with correction, respect, and understanding, individually and personalized.
  • Respect for your privacy by ensuring adequate treatment for incontinence and personal hygiene procedures or others that require invading the nearest protection zone.
  • To maintain the relationships they need, including the right to receive visitors.
  • Confidential treatment of personal data affecting their privacy.
  • To file complaints and suggestions regarding the operation or improvement of services and receive a response.
  • To make decisions on all matters concerning their lives, even at times near death.

Resident Duties

  • Meet and assist in compliance with the internal rules of the center.
  • Exercise behavior based on mutual respect, tolerance, and cooperation, aimed at achieving optimal coexistence, both with the center’s staff and other users.
  • Respect, care for, and assist in the proper maintenance of facilities and equipment used.

General Plan of Action

The general plan of action is the document that displays the services and intervention programs offered by the center to provide comprehensive and personalized attention that will meet the basic needs of the users.

Record Keeping

A record is a document where we write down and leave a written record of our proceedings and findings identified in the course of a procedure or protocol.

Activities of Personal Hygiene

  • Name of the user and any other identification as may be required.
  • Concentration of the activity developed.
  • Date and time of completion of the activity.
  • Recording of observations and incidents, if that is the case.
  • Identification and signature of the persons involved in performing the procedure.

Incontinence and Help to Eliminate

  • Name and ID of users.
  • Specification of the activity.
  • Time of completion of the activity.
  • Recording of observations and incidents, if that is the case.
  • Identification and signature of the persons involved in performing the procedure.

Medication Administration

  • Username.
  • Medication that is administered, route, dose, medicine, fashion management.
  • Identification and signature of the person preparing the medication.
  • Administering the medication.
  • Date and time of administration.
  • Scoring of the incidents occurred.

Falls and Risk of Falls

  • User name.
  • ID, location, time, date.
  • Description of the circumstances of the fall.
  • The consequences of the fall.
  • If it is due to preventable causes, establishing measures to prevent further falls.

Treatment of Pressure Ulcers

  • User name.
  • Cause of injury and date of onset.
  • Evolution stage of the ulcer. Remember the hygiene module.
  • Location of the ulcer or ulcers.
  • Treatment or cures performed, indicating the date and time and the professional who has performed the procedure.
